F2 Sign-off

Towards the end of your F2 Foundation Year you will be expected to arrange a meeting at your Trust with your Foundation Training Programme Director (FTPD). They will review your portfolio and complete a Checklist of Evidence Required to complete the 5.2 Foundation Achievement of Competence Document (FACD). They will then complete your 5.2 Foundation Achievement of Competence Document (FACD). These documents will then be submitted to the Foundation School for the FS Director who will sign you off, subject to successful completion of all requirements of the Foundation Programme.

The Foundation Achievement of Competence Document (FACD) signed by the FTPD and the FS Director certifies that you have met the requirements of your F2 training year and have successfully completed the Foundation Programme.
